The Ontario Student Transcript (OST) is an official record that documents a student's academic progress. It is a component of the Ontario Student Record (OSR) and is created and managed in accordance with the Ministry of Education's OST Manual. Since September 1, 1999, an OST has been established for each student enrolled in an Ontario secondary school course, whether or not the course is being taken for credit.

All graduates receive one diploma and an official transcript at time of commencement. Diplomas and transcripts not distributed at commencement will be held at the secondary school for pick up.

At times, a parent/guardian or adult student may need to request Education Verification from their current or former school for a variety of reasons.

The Ontario Student Transcript (OST) is the official record of a student's academic achievement in an Ontario secondary school. It includes the mark received for each course taken, and indicates whether the student completed the 40-hour community service requirement and passed the Ontario Secondary School Literacy Test. Accessing Student Records

Each student and the parent/guardian(s) of a student who is not an adult (that is, a student who is under the age of 18) may request a meeting to review the contents of the Ontario Student Record (OSR). If the student or the parent/guardian(s) of a student who is not an adult (that is, a student who is under the age of 18) reviews the OSR and wants something removed, they may formally appeal to the principal. Transcript Requests from August 29, 2022 - June 30, 2023

To obtain a student transcript during the school year, please contact the last school you attended. Contact information for each secondary school is available via the drop down Select A School tab at the top of the website.

When contacting the school for your transcript, please provide your full name (if you have changed names, please specify your birth name), and your date of birth.

Do you need someone else to pick up your transcript?

If you are requesting that your transcript be picked up by someone other than you, please complete the Student Transcript Authorization form.

To request your OST for applying to an Ontario college or university:

A online high school transcript request service is available to former LDSB students who are applying to an Ontario College through the Ontario College Application Service (OCAS) or an Ontario University through the Ontario Universities' Application Centre (OUAC).

To use this service, you must:

  • Be a past student of the Limestone District School Board.

  • Apply to a minimum of one Ontario college or university.

  • Pay the applicable application fee.

Note: You only need to request one high school transcript during the application process. Transcripts will be forwarded to all of your college/university choices.

Why use this online service?

  • The college or university to which you applied will receive your transcripts faster.

  • You will be able to track the status of your application and transcript request(s) online.

To order your transcript online, go to ontariocolleges.ca to apply to an Ontario college and ouac.on.ca to apply to an Ontario university.

Over the course of the school year as part of its mandate to educate its students, the Limestone District School Board under the authority of the Education Act, as amended, will collect personal information about each student from both the student and his/her parent(s) or guardian(s). The information collected may be written, oral or visual. This personal information will be used to register and place the student in a school or for a consistent purpose such as the allocation of staff and resources and to give information to other employees to carry out their job duties.

In addition, the information may be used to deal with matters of health and safety or discipline and that may be required to be disclosed in compelling circumstances or for law enforcement matters or in accordance with any other Act. The information will be used in accordance with the Education Act, the regulations, and guidelines issued by the Minister of Education governing the establishment, maintenance, use, retention, transfer and disposal of pupil records.
